All persons (primary account holder) with a valid SSN or tax id number residing in an approved plan state between the ages of 18-64 are eligble to enroll in the PatriotCare Basic or PatriotCare Premier plans; as well as dependent spouses age 18-65; and dependent children ages 0-25.

Persons residing in any foreign country, US Territory, or the following US states are not eligible to enroll in either the PatriotCare Basic or PatriotCare Premier plans.

INELIGIBLE RESIDENT STATES INCLUDE:
Alaska, Connecticut, Hawaii, Maryland, New Jersey, Vermont, and Washington.

Plan Active Date:
All PatriotCare Health Benefit Plans are 1st of the month effective (ex. March 1,2026).

Enrollment Cut-Off Date:
The cut-off date for 1st of the month effective benefits is the 20th of the current month.

Enrollment Date Examples:

• • enrollment date January 5th...Plan effective date is February 1st
• • enrollment date January 21st...Plan effective date is March 1st

First Billing Date:
Your first billing date is the day you enroll in the benefit plan.

Recurring Billing Date:
Recurring monthly billing for your benefit plan begins on the 20th of the month following your effective date; and will reoccur on the 20th of each subsequent month you remain enrolled in the plan.

BIlling Example:

• • Enrolled on January 5th - this is your first billing date...plan effective date is Feb. 1st
• • February 20th would begin your recurring monthly billing for the March benefit coverage period month.
• • Subsequent payment dates would be 3/20; 4/20; 5/20; etc...

Yes. You can upgrade or downgrade your plan benefits once per year without a qualified life event. This benefit level change must be made no later than the 15th of the current benefit coverage period and will not take effect until the 1st of the following month.

Benefit Change Example:

• • Member enrolled in PatriotCare Premium 6000 plan - effective date March 1st
• • June 1st requests plan upgrade to PatriotCare Premium 3000 plan
• • New plan effective date will be July 1st
